Where Ultherapy is used, and where it is not

Ultherapy delivers micro-focused ultrasound to the SMAS layer and the dermis. In the United States it is cleared for lifting the brow (FDA DEN080006) and the neck and under-chin (FDA K121700). In practice the treated areas are the brow, cheeks, jawline and jowls, under-chin and neck. The lower eyelid itself is not treated with Ultherapy.

Jowls and jawline: where the lines are concentrated

Jowls are the SMAS layer sliding downward, which is why a rater-blinded study found measurable lifting of the cheek and neck 90 days after one treatment (Alam et al., 2010). For a patient whose main complaint is jowls, we concentrate the lines along the jawline and the tissue just above it, and keep the mid-cheek count low if the cheeks are slim. That focus, rather than an even full-face grid, is what separates a lifted jawline from a face that simply looks tighter.

Neck: laxity, bands and the under-chin

The neck is a cleared indication and one of the areas where Ultherapy is most useful. Loose skin under the chin, an early double chin and a softening neckline respond to lines placed along the platysma. A neck plan is usually quoted separately from the face because it uses its own line count. If the neck problem is mostly fat rather than laxity, ONDA or fat-dissolving injections are the better first step, and we say so at the consultation.

Eyes: brow lift yes, under the eye no

"Eye Ultherapy" in Korean clinics means treating the brow and the area around the orbit to lift a heavy upper eyelid. That is the original cleared indication. Ultherapy is not placed on the lower eyelid, where the skin is too thin and the eye is too close; under-eye laxity is treated with Thermage Eyes instead. If a clinic offers to treat directly under your eyes with Ultherapy, ask which device and which area they mean.

What Ultherapy Prime changes

Ultherapy Prime is the current Merz platform, cleared by the FDA in 2024 (FDA K233996). It delivers the same micro-focused ultrasound as the original system, so the evidence base carries over (Fabi and Goldman, 2014; MacGregor and Tanzi, 2013). What changes is the physician's side: an updated interface and imaging that make it easier to see the layer before each line, and a faster treatment. For the patient, the practical difference is a shorter session and a plan that is easier to place precisely. How to read a Korean Ultherapy price list

Korean clinics quote Ultherapy by line count, and international patients often see a "price list" with 300-line, 600-line and full-face entries. To compare lists between clinics, and between Korea and home, check five things:

  1. Lines, not areas. A 300-line plan and a 600-line plan are different treatments. Ask which areas each count covers.
  2. Genuine Prime tips. A quote far below what other clinics charge is a reason to ask which device and which tips are used.
  3. Who performs it. Physician-planned and physician-performed treatment is priced differently from delegated treatment. Many chains quote low because a technician places the lines.
  4. Anesthesia. Numbing cream is standard; sleep sedation is quoted separately.
  5. What is not included. A neck plan, or a Thermage add-on, is usually outside the face price.

On the comparison with Singapore or the United States: Korean clinics tend to quote per line rather than per area, and the consumable cost is similar everywhere, so the difference usually lies in physician time and clinic overhead rather than in the device. Physician-planned or factory-style: how to tell

The questions that separate the two are simple. Who examines your face before the plan is set? Who places the lines? How many patients does the clinic treat per hour? When Ultherapy is a waste of money

Honest answer: for some faces it is. Ultherapy is not the right treatment when

  • the skin is loose but the deeper layer is not sagging, where Thermage does the job for less;
  • the cheeks are already slim and the main complaint is hollowness, where lifting makes it worse;
  • the sagging is advanced, where a non-surgical lift will disappoint and surgery is the honest recommendation;
  • the concern is fat under the chin rather than loose skin, where ONDA or injections come first.

We would rather tell you at the consultation that you are not a good candidate than treat you and have you judge Ultherapy by a result it could not deliver.

Reviews and before-and-after photos

Reviews of Ultherapy diverge mostly on two points: whether the face looked gaunt afterwards, and whether the result was visible at all. Both usually trace back to line placement, so when you read a review look for the line count, the areas and the patient's cheek volume. Photographs should be compared at three months or later, in the same lighting and expression. We discuss representative cases at the consultation in relation to your own face rather than as a promise.
