Most Thermage questions we receive from international patients are not about how the device works. They are about whether it is worth it: how long the result really lasts, why it costs more than other tightening treatments, whether it can handle the lower face and neck, what a 40s or 50s face can realistically expect, and how to read the reviews and before-and-after photos you find online. How long Thermage FLX lasts, realistically

Thermage works in two phases. Heat contracts existing collagen on the day, which is why the skin feels firmer immediately. The larger change comes from new collagen, which builds over two to six months. A prospective clinical trial of the current generation of monopolar radiofrequency, the FLX generation, measured improvement in facial laxity over the months after a single treatment (Angra et al., 2021, Dermatologic Surgery), and a Korean histometric study showed the structural change in the dermis that underlies the result (Suh et al., 2020).

In practice, the result is commonly maintained for around a year, and many patients choose to repeat yearly. Three things shorten or lengthen that:

  • Age and starting laxity. Younger skin with mild laxity holds the result longer; more advanced laxity in the 50s tends to need yearly treatment or a combination.
  • Skin thickness. Thicker skin responds and holds well; very thin skin shows fine lines again sooner.
  • Lifestyle. Sun exposure, smoking and rapid weight change all shorten collagen gains.

What Thermage does not do is stop aging. The fair way to describe it is that it sets the clock back by a margin that lasts about a year, and repeated treatments maintain that margin.

Why Thermage costs what it does

Patients often compare Thermage with cheaper radiofrequency or ultrasound treatments and ask why it is priced higher. The cost is mostly in the consumable and the time:

  1. The tip is single-use and priced per shot count. Every Thermage FLX treatment uses a new tip with a fixed number of pulses, 600 for a typical face, 900 when the neck is added, and a separate eye tip. That tip is the largest part of the cost and cannot be reused.
  2. Device generation. FLX tips cost more than tips for older CPT or NXT systems. The FDA cleared the FLX system in 2017 (FDA 510(k) K170758); ask which generation a quote refers to.
  3. Tip size is part of the plan. A Korean split-face study compared rejuvenation results by radiofrequency tip size (Yang et al., 2024), which is why the tip and shot count are chosen for the area rather than fixed.
  4. Physician time. Distributing 600 or 900 pulses by area, with passes adjusted where skin is loose, takes longer than a single protocol. A plan designed and performed by the Medical Director is priced differently from delegated treatment.
  5. Anesthesia and packages. Local or sleep anesthesia is separate; packages with Ultherapy or skin boosters sometimes reduce the shot count, so ask what is inside.

Neck and jawline: treating the lower face together

The lower face and neck are where Thermage is most often combined into one plan. With the 900-shot tip, the jawline, the under-chin area and the neck can be treated in the same session, which matters for crepey neck skin and a softening jawline that are losing firmness together. Monopolar radiofrequency has been evaluated for energy delivery and skin tightness in the face and neck (Choi et al., 2025, Lasers in Medical Science). If the neck has actually loosened, with visible bands or a double chin, Thermage alone will firm the surface but not lift the structure; that is where Ultherapy on the neck is added. Cheek sagging in your 40s and 50s: what Thermage can and cannot do

In the early 40s, cheek laxity is often mostly a firmness problem, and Thermage on its own gives a visible, natural improvement. By the late 40s and 50s, sagging usually involves the deeper layer as well. Thermage will make the skin firmer and smoother, but it will not reposition a cheek that has descended. For that face, the honest plan is Ultherapy for the deeper layer with Thermage for the surface, or, if cheeks have also lost volume, a volume treatment planned alongside.

We would rather tell you at the consultation that Thermage alone is not enough than have you judge the result against an expectation it could not meet.

Reading reviews and before-and-after photos

Online reviews of Thermage range from "no difference" to "dramatic", and most of the gap comes from three things: which generation was used, how many shots were delivered and where, and whether the patient was a good candidate in the first place. When you read a review, look for the tip and shot count and the patient's age. When you look at before-and-after photos, look for the same lighting, angle and expression, taken at least three months after treatment; photos taken on the day mostly show swelling.
